Interested in supporting the future of the Heartfull Collaborative program?
The Heartfull Collaborative program is a specialized offering of Sinai Health’s Temmy Latner Centre for Palliative Care. The program focuses on patients living with advanced heart failure, and supports research efforts to improve the care provided to these individuals.
The Heartfull Collaborative program started as a small Sinai Health initiative, but thanks to interest and support, it has expanded to support palliative care for heart failure patients from across the Toronto region.
Sinai Health now aims to expand the Heartfull Collaborative program even further, helping to share and expand the model to other institutions and cities.
