Eligibility
To be eligible to enter and win, the entrant must be of legal drinking age (19 years or older) at the time of entry and a resident of Ontario and not be a) an employee of Sinai Health System(“SHS”) or Sinai Health System Foundation (“SHF”), the Contest Prize suppliers (“Prize Sponsors ”), their respective advertising or promotional agencies, and b) a member of the immediate family, or a person who is domiciled with any of the above persons.
How To Play
To enter, visit www.supportsinai.ca/mothersday between Wednesday, March 31, 2021 at 9:00 am EST, and Thursday, May 6, 2021 at 3:00pm EST (the “Contest Period”). Click on the identify link and fill in all required fields on the Online Entry Form and share a story and/or photo of a mother or mother figure in your life, and why they are a hero (the “Entry Form”) and accept all Official Rules. Upon clicking the “Submit” button, a confirmation message will confirm the successful receipt of an entry submitted. Only one entry may be submitted per person/email address.

Contest Period
The Contest runs from Wednesday, March 31, 2021, at 9:00 am EDT to Thursday, May 6, 2021, at 3:00 pm EDT. All times refer to Eastern Daylight Time (“E.D.T.”) throughout the contest.

The prize must be accepted on an “as is” basis. No substitution prize will be awarded. However, the Prize Sponsor reserves the right to substitute a prize of equal or greater value without notice. If the selected entrant cannot use or chooses not to use any portion of the prize, that portion of the prize will be deemed to be forfeited and no substitution or other compensation will be awarded. Please note Prize does not include any travel to/from the Fairmont Royal York Hotel. Prize is not transferable or otherwise redeemable for cash. Prize will only be released to the verified winner. Return of any prize/prize notification as undeliverable may result in disqualification and selection of an alternate winner.
Odds of winning depend on the number of eligible entries received during the Contest Period.
Prize Draw
A random draw from all eligible entries will be held on or about May 6, 2021, in Toronto, Ontario by the CEO of SHF and is final. The chances of winning depend on the total number of eligible entries received. In order to win, the selected entrant must have complied with the rules and will be required to correctly answer, unaided, a time-limited mathematical skill-testing question to be administered by telephone at a pre-determined mutually convenient time. A selected entrant may be disqualified and the prize awarded to an alternate entrant (selected and subject to disqualification in the same manner) if: (a) the selected entrant cannot be reached within five (5) business days of the draw; (b) any prize/prize notification is returned as undeliverable; (c) the selected entrant fails to prove eligibility or to otherwise comply with these official rules; (d) the selected entrant fails to answer the skill-testing question correctly; or (e) the selected entrant fails to return the Declaration and Release (defined below) within the specified deadline. No more than one prize per person or per household or per emailing address will be awarded.
General Conditions
Before being declared a winner, the selected entrant will be required to sign a standard document confirming compliance with the contest rules, acceptance of the prize as awarded and a release of liability (“Declaration and Release”) in a form acceptable to the Prize Sponsor and SHF, releasing SHF,SHS, the Prize Sponsor and all of their respective directors, officers, employees, agents and affiliates (collectively, the “Releasees”) from any liability of any kind in connection with this Contest or the prize being awarded. Declaration and Release documents must be returned within the time period indicated in the documents or the prize may be forfeited. Entrants assume all liability for injuries caused or claimed to be caused by participation in this Contest, or the use or misuse of any prizes(s) awarded therein. The winners agree to the use of their names and likeness in any related publicity without compensation. The contest is subject to all applicable federal, provincial and municipal laws. By entering the contest, entrants agree to abide by the contest rules and decisions of SHF, which are final. No correspondence will be entered into except with the selected entrants, who will be contacted by mail, email or phone.
Release
BY ENTERING THIS CONTEST, YOU RELEASE SHS, SHF, THE PRIZE SPONSOR AND THEIR RESPECTIVE DIRECTORS, OFFICERS, EMPLOYEES, AGENTS AND CONTRACTORS FROM ANY AND ALL LIABILITY ARISING OUT OF OR IN CONNECTION WITH THIS CONTEST AND THE AWARDING OF ANY PRIZE, NOW OR IN THE FUTURE.
Disclaimer
All entries become the property of SHF upon receipt and will not be returned. The decisions of SHF will be final and binding on all entrants. Neither SHF nor the Prize Sponsor assumes any responsibility for lost, stolen, delayed, damaged or misdirected entries or for any failure of the contest website during the promotional period, for any problems or technical malfunction of any communications network or lines, computer on-line systems, servers, access providers, computer equipment, software, failure of any e-mail or entry to be received by SHF on account of technical problems, human error or traffic congestion on the Internet or at any website or any combination thereof including any injury or damage to an entrant’s or any other person’s computer related to or resulting from playing or down loading any material in the promotion. SHF reserves the right, in its sole discretion, to cancel or suspend this contest should a virus, bug or other cause beyond the reasonable control of SHF corrupt the security, fairness, integrity or proper administration of the contest. Any attempt to deliberately damage any website or to undermine the legitimate operation of this promotion is a violation of criminal and civil laws, and should such an attempt be made, both the Prize Sponsor and SHF reserve the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution. Contest entries are subject to verification and will be declared invalid if they are illegible, mechanically reproduced, not legitimately obtained, mutilated, forged, falsified, altered or tampered with in any way. Subject to applicable law, SHF reserves the right to withdraw, amend or terminate this Contest at any time without prior notice and reserves the right to cancel this contest at any time because of any printing or other error, or if it suspects or finds evidence of tampering with any part of the contest. In the event of a dispute as to who submitted an entry, the entry shall be deemed submitted by an “authorized e-mail account holder” and entries will be declared made by the authorized e-mail account holder of the e-mail address submitted at the time of entry. “Authorized e-mail account holder” is defined as the natural person who is assigned to an e-mail address by an Internet access provider, on-line service provider, or other organization (e.g., business, educational institution etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. The potential winner may be requested to provide SHF with proof that the potential winner is the authorized account holder of the e-mail address associated with the potentially winning entry.
CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE OR UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST IS A VIOLATION OF CRIMINAL AND FEDERAL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SHF RESERVES THE RIGHT TO SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON (S) RESPONSIBLE FOR THE ATTEMPT TO THE FULLEST EXTENT PERMITTED BY LAW.
Miscellaneous: By entering the contest, you agree to abide by and be bound by these contest rules and the decisions of SHF and the Prize Sponsors, which are final. By entering, each winner agrees that SHF and Prize Sponsors may use their name, address, comments, photography and likeness in any matters related to this contest, including any advertising or publicity, without compensation or further consent. SHF and Prize Sponsors accept no responsibility for losses, damages or claims caused by or resulting from any prize. Correspondence will be entered into only with prize claimants. SHF and the Prize Sponsors reserve the right to substitute any prize with one of equal or greater value.
This contest is subject to all applicable federal, provincial and municipal laws.
The contest is conducted by SHF and the Prize Sponsors. The contest is intended for Ontario residents only and is governed by Canadian law.
